Job Summary
Conduct enrollment procedures, gather child health and development information, and perform home visits to assess family strengths and needs. Provide direct health and social services by administering medication per protocols, conducting screenings, and connecting families to community resources for emergency assistance, education, and medical care. Coordinate case management meetings, develop individual Care Plans, and observe classrooms to ensure safe environments while compiling accurate data and monthly reports. Foster community understanding of the program's mission and empower parents to influence local systems of care.
Required Qualifications
- High school diploma or GED
- A credential, certificate or degree in social work, human services, family services, counseling or related field
- 1 year of work experience in a health and/or social services field or in a position that combines administrative and customer support duties
- Must complete OCDC's Family Advocate Certificate Level 1 within 12 months of hire
- 2 years of work experience in an environment working with children and/or families
- Must complete OCDC's Family Advocate Certificate Level 1 within 12 months of hire
