Job Summary
Provides program case management services to children and families, including enrollment procedures, health screenings, home visits, and connecting families to community resources; collaborates to develop Care Plans and support parent involvement, aligned with Head Start Performance Standards; requires a combination of education and/or experience with a certificate within 12 months of hire, plus completion of the OCDC Family Advocate Certificate Level 1; located in Independence, OR.
Required Qualifications
- High school diploma or GED AND a credential, certificate or degree in social work, human services, family services, counseling or related field OR 1 year of work experience in a health and/or social services field or in a position that combines administrative and customer support duties AND Must complete OCDC’s Family Advocate Certificate Level 1 within 12 months of hire OR 2 years of work experience in an environment working with children and/or families AND Must complete OCDC’s Family Advocate Certificate Level 1 within 12 months of hire.
