Job Summary
Serve as the primary point of contact between families, community partners, and state agencies, following the established family map to link to services, provide psycho-education and community education, advocate for families, and support youth and their families. Responsibilities include advocating and representing families, developing a family-driven approach, coordinating with program staff, maintaining a regular weekly schedule, attending trainings (Risking Connection, DMH trainings, and other required sessions), conducting outreach to market the Link Program, documenting all services within a 48-hour timeframe, and carrying out administrative duties while maintaining cultural competency. Requires ability to connect families with information, referrals and supports, provide crisis assistance as needed, participate in team meetings, build professional partnerships, and perform quality assurance and program improvement activities.
Required Qualifications
- High School Diploma or GED equivalent
- Ability to articulate concepts in a clear manner to families
- Ability to listen and problem solve with families
- Excellent organizational skills
- Experience in diverse communication styles
- Familiarity with MDHS, TANF, and other state services and navigation
