Job Summary
Conduct initial family needs assessments and develop individualized family partnership agreements grounded in the Parent, Family and Community Engagement Framework. Perform regular home visits and maintain accurate documentation of interactions, referrals, and follow-ups in the Child Plus data system. Advocate for families to access community resources for housing, employment, education, and health care while monitoring progress toward goals and ensuring compliance with federal and state regulations. Collaborate with teaching staff and health teams to support child development and facilitate family participation in engagement activities.
Required Qualifications
- Associate degree in Social Services or related field
- Two (2) years' experience in casework delivery to families with young children
- Intermediate computer literacy including online data systems and Microsoft Office
- Experience working with young children and their families
- Knowledge of and sensitivity to the unique challenges faced by families living in poverty
- Valid Driver's License
- Insured vehicle (100/300 liability insurance coverage)
- Acceptable alternative to driving vehicle
- Driving record check
- Criminal record check
- Finger printing
- Initial physical examination record
- TB screening
- Ability to work flexible hours
- Reliable source of transportation
- Physical strength/endurance/mobility to perform in a standard daily required work routine
- Ability to maintain and model good hygiene
