Job Summary

Coordinate services and resources across healthcare, social services, and community organizations for individuals navigating crisis or instability. Conduct comprehensive assessments to capture participant needs, barriers, and aspirations, then partner with them to develop individualized, goal-oriented action plans. Maintain consistent communication to monitor progress, troubleshoot barriers, and serve as a trusted advocate in settings such as court appearances and social service appointments. Accurately record case notes and track outcomes while building relationships with local agencies to expand support networks. Work Monday through Friday, adhering to ethical standards and confidentiality requirements.

Required Qualifications

  • A High School Level education, or its equivalent
  • A satisfactory equivalent of education, training, and experience in related fields and/or educational disciplines, sufficient to qualify for the position's requirements
  • A Christian in submission to the authority of the Scriptures and the Holy Spirit, as evident in lifestyle
  • Subscribes to and is in complete agreement with the Church of the City's Statement of Faith
  • Represents and upholds Church of the City's mission, vision, and culture by demonstrating a strong work ethic, positive attitude, and learning posture
  • Willing to become a stakeholder of COTC within six months of hire
  • Competency and demonstrated experience in effective communication
