Job Summary
Provides case management services, performs home visits, and networks with community agencies to support families in need. Develops individualized action plans, implements seasonal initiatives such as Christmas Distribution and components of the School Readiness Program, and maintains required records and reports for the program. Required Qualifications
- High School Diploma or Equivalent In General Studies
- 1+ years of experience in a related field or a combination of education and experience
- Valid Kentucky Driver's License and maintain a Safe Driving Record
- CAP Driving Certification (or ability to obtain within first 90 days)
- Ability to drive a personal vehicle on CAP business with adequate insurance coverage
- Lift and physical capability to perform duties (lift 50 lb; 25 lb overhead)
