Family Advocacy Case Manager

Job Summary
Maintain a caseload of shelter families by building rapport, assessing individual needs, and implementing action plans focused on stable housing, income, and well-being. Conduct initial orientations, follow up on goals, and utilize strength-based practice to support residents through their stay. Provide direct advocacy by making phone calls, attending meetings with service providers, and addressing concerns to create solutions. Assist families with employment resources, material needs, and onsite services while coordinating mental health referrals and tracking records in the CRM and HMIS systems. Utilize translation services and adhere to trauma-informed care and data privacy guidelines.
Required Qualifications
- Valid, Class D driver's license
- Two or more years of experience providing culturally responsive services to individuals and families from diverse backgrounds and lived experiences
- Two or more years of experience working with families or individuals experiencing homelessness, housing instability, or other barriers to stability, or in a closely related human services field
- Highly competent with Microsoft software including Outlook, Word, Excel, and the Office365 suite of applications including Microsoft Teams and SharePoint
- This is an in-person role without the ability to work remotely
- Rare weekend duties may be required
- Advanced noticed would be given
Desired Qualifications
- 4 year degree in social work, psychology, sociology, human services or closely related field
- Relevant experience will also be considered
- Experience using Salesforce and the Homeless Management Information System
