Job Summary
Service Facilitator role within the Families First Program providing intensive case management to children and families with severe emotional disabilities (SED). Uses wraparound service delivery to identify needed services and develop multi-disciplinary treatment plans, collaborating with case managers, family advocates, mental health providers, AODA providers, school staff, juvenile justice workers, and child welfare workers. Responsibilities include completing Strengths and Needs assessments, creating and updating individual treatment plans, building a family support team, facilitating team meetings, developing crisis/safety plans, intervening as an advocate in emergencies, attending IEP meetings and related appointments, and transporting clients as needed. Salary = $23.50/hour. Location is Kenosha, WI with hybrid scheduling and local travel; schedule Mon-Fri 8:00am-5:00pm with potential hour-flex to meet client needs; must have reliable transportation and a valid driver’s license.
