Job Summary
Facilitate the SNAP curriculum with children and families, conducting participant intakes, screenings, and assessments while maintaining ongoing engagement to support positive behavioral change. Collaborate with SNAP coordinators, school personnel, and community partners to coordinate services, assist with recruitment and retention, and provide referrals to appropriate resources. Maintain accurate documentation including case notes, attendance logs, and reports; participate in staff meetings, supervision, and program planning. Travel throughout the service area to schools, homes, and agency locations to deliver quality service delivery. Adhere to all agency policies, laws, and confidentiality standards while working flexible hours including evenings.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Counseling, Criminal Justice, Education, Human Services, or a related field
- Must successfully obtain Criminal History Clearance from the Pennsylvania State Police and Child Abuse Clearance from the Pennsylvania Department of Human Services and Fingerprint Based FBI Clearance
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program
- Ability to transport oneself as program needs dictate and maintain a valid driver's license
- Ability to travel throughout the service area to schools, homes, community sites, and agency locations
- Ability to work independently while functioning effectively as part of a multidisciplinary team
- Ability to work flexible hours, including evenings, to accommodate participant and family schedules
Desired Qualifications
- Experience working with children, adolescents, and families
- Experience facilitating groups, providing behavioral interventions, or working with school or community-based settings
- Strong verbal, written, organizational, and interpersonal communication skills
- Culturally responsive and sensitive to the diverse needs of the community
