Facilitator - Safe at Home/CSED Wraparound Program

Job Summary
Wraparound Facilitator role providing family-driven, client-centered services across in-home and community settings. Responsible for maintaining a caseload of 8-10 families, leading the family team, delivering assessments and individualized family planning, intensive case management, and oversight of wraparound meetings and supports. Facilitates a transition from formal services to natural and community supports, participates in on-call rotation, conducts community outreach, and coordinates services across multiple environments. The role requires travel between locations, a client-centered and trauma-informed approach, and collaboration with a multidisciplinary team to develop and monitor family plans, ensure timelines are followed, and document outcomes. Strong organizational, communication, and documentation skills, plus the ability to navigate software systems and manage invoicing and contracts for the team, are essential. The position emphasizes adherence to wraparound principles, data tracking for state reporting, and contributing to program development, while embracing Necco’s culture and core virtues.
Required Qualifications
- 21 years of age
- Bachelor’s degree; Master’s degree preferred
- Licensed Social Worker or eligible for licensure in West Virginia preferred, not required
- Minimum of 2 years experience working with families and/or children with emotional or behavioral problems
- Valid driver’s license
- 100/300/100 Auto Insurance Coverage
- Training and Travel willingness
- Excellent organizational skills
- Excellent written and oral communication skills
- Successful completion of all required criminal background checks
- Proficiency with software programs (e.g., Microsoft Office, Database, Electronic Health Record, Dashboard)
