Job Summary
Coordinate all after-hour and weekend home health and hospice referrals, including scheduling patient visits based on physician orders and managing intake information. Answer incoming calls from patients, families, and referral sources while communicating with the Extended Hours Triage RN, field clinicians, and other stakeholders. Enter and verify insurance data, ensure compliance with regulatory guidelines, and escalate issues to clinical leaders as needed. Work Saturdays and Sundays from 8:30 AM to 4:30 PM, with four holidays annually, remotely from home while residing near the Ridgefield, CT office for required onsite training. Maintain a strong understanding of payer requirements and contribute to quality improvement initiatives.
Required Qualifications
- Minimum of 2 years' experience in healthcare
- Familiarity with medical terminology
- Must be available to work all weekends and four holidays per year
- Must reside within a reasonable commuting distance of RVNAhealth's Ridgefield, CT 06877 office
- Must be able to report onsite for required training, meetings, or other business needs as requested
- Must submit to a pre-employment physical/drug screen and background check
