Extended Foster Care Success Coach

Job Summary
Develop and maintain supportive, non-judgmental relationships with young adults in foster care to encourage voluntary participation in Arizona's Extended Foster Care program until age 21. Conduct weekly check-ins with supervisors and complete monthly clinical supervision, while managing a caseload of up to 20 individuals across Arizona. Perform at least 35 in-person monthly visits to assist with housing stability, goal setting, and crisis support, ensuring all contacts are documented within two business days. Collaborate with community partners to address physical and behavioral health needs, organize Age of Majority meetings, and complete transition plans every six months. Report directly to an EFC Program Manager, requiring a valid driver's license, CPR certification, and clearance of the Central Registry.
Required Qualifications
- Flexible, solutions-oriented attitude
- Knowledge of trauma-informed care and healing-centered approaches
- Understand the community resources and providers
- Bachelor's or associate degree in an appropriate discipline
- Skills acquired through alternative routes, such as relevant job training, community college attendance, military service, or an apprenticeship
- At least 26 years of age
- Current CPR certification
- A valid driver's license that has not been suspended or revoked due to a moving violation within the most recent thirty-nine (39) months and valid car insurance
- Cleared Central Registry Background Check
- A Level One fingerprint clearance card
- Access to the DCS Guardian system
- Ability to register and take all necessary computer-based training prior to receiving access to the ADCS Child Welfare Case Management System
