Extended Care Registered Nurse (RN)

Provide skilled nursing services in after-hours and weekend care on a 7-on/7-off schedule, including nursing and intake/referral calls, patient assessments (phone and in-person), and coordination of scheduled and as-needed visits to manage complex conditions and prevent hospitalizations. Collaborate with an interdisciplinary team to evaluate needs, plan and deliver care that enhances independence and autonomy for older adults. Responsibilities include nursing procedures, monitoring patient conditions, educating participants and families on care needs, triaging calls, maintaining timely documentation, and adhering to organizational policies, procedures, and regulatory requirements; participate in quality improvement and ongoing team collaboration.
