Experienced RN – Pediatric Skilled Nursing | Trach & Vent Care | Lorain, County | Morning Shift

Conduct comprehensive patient assessments, develop individualized Plans of Care, and deliver skilled nursing services including wound care, medication administration, and pediatric care. Monitor patient conditions, maintain accurate clinical documentation, and coordinate care with physicians, families, and interdisciplinary teams. Perform supervisory visits, complete caregiver competency evaluations, and participate in quality assurance and chart review activities. Support new client admissions, transitions of care, and ensure compliance with Joint Commission standards. Requires current unrestricted Ohio RN license, 3 years clinical experience, and ability to travel throughout the West Cleveland service area.
