Job Summary
Conduct thorough evaluations to assess patients' needs, including reviewing medical histories and implementing specific assessments tailored to individual conditions. Develop and explain detailed treatment plans, ensuring patient and family understanding and involvement in goal setting. Implement tailored treatment protocols based on evaluation findings and regularly review patient progress, adjusting care plans as necessary. Collaborate with multidisciplinary healthcare teams to optimize patient outcomes, and document any changes in patients' conditions that may necessitate adjustments in treatment. Manage discharge planning by providing clear follow-up care instructions alongside detailed documentation to track patient progress accurately. Contribute to departmental goals by actively participating in marketing initiatives and community outreach programs. Pursue continuous professional education and serve as a clinical mentor to students and junior staff. This full-time role is located in Branford, Connecticut, with an annual salary ranging from USD 100,000 to 120,000.
Required Qualifications
- A Bachelor's Degree in Physical Therapy or a Doctorate degree in Physical Therapy (DPT) from an accredited institution
- A valid state license to practice as a Physical Therapist in Connecticut
- Current American Heart Association's BLS for Healthcare Providers CPR certification
- Dedication to continuous professional development and improving patient care outcomes
