Job Summary
Direct physical therapy treatment plans to prevent disabilities, relieve pain, and enhance muscle function for patients recovering from surgery, injury, or chronic conditions. Conduct thorough diagnostic assessments of muscular, neurological, and joint function while analyzing home environments to recommend independence-focused strategies. Oversee Physical Therapy Assistants to ensure care plan implementation aligns with prescribed objectives. Deliver services including manual therapy, exercise regimens, and physical agents in compliance with medical prescriptions. Document clinical records, progress notes, and discharge summaries per OASIS standards. Participate in interdisciplinary conferences to update comprehensive care strategies and instruct clients and families on therapeutic device use.
Required Qualifications
- A completed Doctorate in Physical Therapy (DPT) from an accredited program
- A valid state licensure as a Physical Therapist in good standing
- A minimum of one (1) year of clinical experience as a physical therapist
- Current CPR certification
- An aptitude for creative problem-solving and informed decision-making
- Exceptional interpersonal and communication skills

- Experience in home healthcare delivery models and familiarity with community resources for rehabilitation
- Proficiency in electronic health record (EHR) systems
