Job Summary
Conduct comprehensive patient assessments, develop individualized treatment plans, and utilize standard physical therapy procedures to treat injuries and disabilities in home care settings. Educate patients and caregivers on condition management while collaborating with physicians and nursing staff to optimize outcomes. Maintain accurate patient documentation, participate in quality improvement audits, and assist with operational tasks like scheduling and inventory management. Develop promotional strategies to increase community referrals. Work onsite primarily in patients' homes Monday through Friday, 3 pm to 7 pm, with a 4-hour Friday shift.
Required Qualifications
- A current state licensure as a Physical Therapist
- A Bachelor's degree in Physical Therapy from an accredited institution
- Demonstrated clinical experience in a healthcare setting, particularly in home care or community-based therapy
- Strong interpersonal skills to effectively communicate with patients, families, and other healthcare professionals
- Ability to perform physical tasks related to physical therapy procedures, including lifting and manual dexterity
- Experience with a diverse patient population, particularly in home care settings
- Knowledge and understanding of community resources available for patient referrals and support
- Proficiency in documentation software and electronic health record (EHR) systems
- Ability to train and supervise physical therapy assistants and support staff effectively
