Job Summary
Conduct thorough patient evaluations, including medical history reviews and assessments, to develop tailored treatment plans for orthopedic conditions. Implement individualized protocols based on patient progress while collaborating with healthcare providers to adjust care and manage referrals. Prepare comprehensive discharge plans, maintain accurate documentation, and communicate findings to patients and families for goal-setting. Manage time effectively to meet productivity targets while engaging in marketing and community outreach initiatives. Participate in ongoing professional education and mentor interns and students to foster continuous learning within the home care setting.
Required Qualifications
- Licensed Doctor of Physical Therapy (DPT) from an accredited program
- Minimum of 2 years' clinical experience as a physical therapist
- Current CPR certification
- Ability to demonstrate strong interpersonal skills
- Effective communication capabilities
- Proficiency in electronic health record (EHR) systems
