Job Summary
Conduct comprehensive assessments and evaluations of patients with orthopedic conditions, developing personalized treatment plans that align with individual goals. Implement therapeutic interventions including exercises, manual therapy, and modalities to enhance healing and restore function. Monitor patient performance, provide motivation, adjust treatments, and document progress thoroughly in the electronic health record system. Educate patients and families about diagnoses, treatment options, and preventive exercises to promote self-management. Participate in multidisciplinary team meetings to discuss patient treatment plans and outcomes. Work directly in patient homes as part of the Home Care division, serving diverse populations with post-surgical recovery, sports injuries, and chronic musculoskeletal conditions.
Required Qualifications
- A minimum of a Bachelor's Degree in Physical Therapy from an accredited institution
- Current valid state licensure/certification as a Physical Therapist in New Jersey
- Minimum of 2 years of experience in physical therapy
- Experience with orthopedic conditions
