Job Summary
Conduct thorough evaluations, create customized treatment plans, and implement evidence-based therapeutic strategies for patients recovering from surgery, managing chronic conditions, or rehabilitating sports injuries. Provide physical therapy across specialties including orthopedics, pediatrics, neurology, and sports medicine within varied home settings. Administer skilled sessions focused on home-based rehabilitation, educate patients and caregivers on therapeutic exercises and lifestyle modifications, and collaborate with multidisciplinary teams to optimize outcomes. Deliver exceptional customer service while meeting utilization goals and participating in clinic performance initiatives. Work Monday through Friday for 40 hours onsite at patient homes.
Required Qualifications
- A Doctorate in Physical Therapy (DPT) from an accredited institution
- State licensure as a Doctor of Physical Therapy (DPT) recognized by the Physical Therapy Examining Committee
- A minimum of two years of practical experience in an active therapy setting
- Certification in CPR, with current knowledge of emergency procedures
- Strong written and verbal communication skills to connect effectively with patients and their families
- Proficient computer skills, including familiarity with electronic health records (EHR)
- Ability to develop and maintain meaningful patient relationships to inspire and motivate them towards their recovery goals
- Onsite presence at patients' homes
- Working Monday to Friday over 40 hours
