Conduct assessments, design tailored reablement plans, and guide individuals to regain independence after hospital stays using person-centred practice. Act as a trusted advisor on equipment, safe practice, and decision-making while supporting colleagues and contributing to service improvements. Manage your own schedule across varied caseloads in homes, hospitals, and community settings with ongoing professional support from the Principal Occupational Therapist. This role offers autonomy and flexibility within the Reablement Team at Wiltshire Council, where you will help people rebuild skills and live independently.
