Job Summary
Perform skilled nursing visits in patients' homes, conducting thorough assessments and monitoring condition changes. Develop and implement individualized plans of care under RN supervision while accurately completing electronic documentation. Educate patients and caregivers on disease management, medications, and home safety, and communicate effectively with physicians and the interdisciplinary care team. Ensure compliance with agency policies and Medicare guidelines. Requires current Texas LVN license, minimum one year of home health experience, CPR certification, and reliable transportation. Flexible scheduling available with competitive pay.
Required Qualifications
- Current Texas LVN license in good standing
- Minimum 1 year of recent home health experience
- Strong clinical assessment and documentation skills
