Job Summary
Conduct thorough evaluations of patients to assess physical function, mobility, and rehabilitation needs, setting specific goals aligned with physician recommendations. Develop and implement individualized treatment plans using evidence-based practices tailored to each patient's unique situation. Document all interactions meticulously, including evaluations, treatments, progress notes, and discharge summaries. Supervise and mentor Physical Therapy Assistants while collaborating regularly with physicians, nurses, and other specialists to ensure cohesive care. Engage in continuous education and professional development activities to enhance clinical skills. Work onsite within patients' homes, primarily serving adult and elderly populations with conditions affecting mobility, post-operative recovery, chronic pain, and disabilities.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution
- Current licensure as a Physical Therapist in the state of Virginia
- Strong verbal and written communication skills
- Proficiency in computer systems and documentation
Desired Qualifications
- A minimum of two years of experience in a physical therapy setting
- Experience in home health settings or with geriatric populations
- Familiarity with utilizing therapy technologies and methods to maximize patient outcomes
