Executive Director of Care Management

Executive Director of Care Management leads the Case Management program within an acute hospital setting. Oversees daily operations, directs core Case Management functions, and coordinates across interdisciplinary teams and stakeholders to achieve clinical, service, and financial outcomes. Requires RN with acute hospital experience and a BSN; a Master’s in Nursing is preferred, with CCM/ACM certification preferred within 2 years of hire. Responsibilities include resource allocation, staffing productivity and quality goals, adherence to care coordination and discharge planning processes, process improvement, and performance evaluation through data analysis and information technologies.
