Job Summary
Coordinate MDS assessments and Care Plans for LTC residents; manage the RAI process to ensure regulatory compliance and accurate documentation, optimizing reimbursement. Lead daily admission and care planning meetings; communicate acuity changes to leadership. Oversee data transmission of MDS information to state/federal agencies, conduct regular medical-record audits to validate MDS coding, and train facility staff on MDS terminology, documentation requirements, and reimbursement regulations to enhance clinical data integrity.
Required Qualifications
- Current Registered Nurse (RN) license in New Jersey (Required)
- 2+ years of clinical nursing experience in a long-term care (LTC) setting
- Proven experience with MDS 3.0 completion, RAI processes, and Medicare/Medicaid reimbursement
