Job Summary
ESSHI Care Navigator acts as a liaison between tenants and program support services to ensure quality services and wellness. Responsibilities include greeting tenants, orienting them to the unit, reviewing eligible services, helping identify needs for the unit, developing and updating individualized care plans within 5 days of admission, coordinating with Presbyterian Residential Care Management Agency, conducting monthly unit checks, monitoring Medicaid status and recertification needs, maintaining documentation, advocating for additional services, collaborating with ESSHI team members, attending team meetings, and taking after-hours on-call shifts. Requires collaboration with service providers, strong documentation, travel, and independent decision-making. Bachelor’s degree in a related field and 1-2 years in Human Services (geriatric focus), basic computer skills, fingerprint criminal history check, and valid NYS driver’s license are required.
Required Qualifications
- Bachelor’s degree in psychology, sociology, human services or related field
- 1-2 years’ experience in Human Services, primarily Geriatric
- Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel)
- This position does have regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check
- Travel is required. Must have a valid NYS Driver’s License and reliable transportation
- Must possess the ability to make independent decisions when circumstances are warranted
