Job Summary
Collaborate with multi-disciplinary teams to optimize drug therapy and provide direct patient care, including treatments assessment, medication management, and daily clinical evaluations. Review patient drug regimens for appropriate indication, dosing, allergy verification, and potential interactions while ensuring documentation of recommendations and medication reconciliation. Respond to level 1 trauma and code blue emergencies, providing accurate drug information to physicians, nurses, and families. Monitor polypharmacy and high-dose dosing to prevent adverse effects, develop clinical protocols, and facilitate discharge planning. Process orders and prescriptions for medication and pharmaceutical supplies while managing drug utilization cost-effectively.
Required Qualifications
- Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND completion of ASHP-accredited PGY2 in Emergency Medicine Pharmacy or Critical Care Pharmacy
- Completion of ASHP accredited PGY1 Pharmacy Practice Residency AND 3 years of practical clinical practice experience in Emergency Medicine with BPS board certification
- American Heart Association Basic Life Support (BLS) certification
- American Heart Association Advanced Cardiovascular Life Support (ACLS) certification
- Meets standards of an ASHP-Qualified Residency Preceptor within one year of employment
- Bachelors degree in pharmacy
- Minimum of one year of pharmacy experience within the last three years
