Job Summary
Conduct individualized assistance for patients, families, and caregivers to overcome barriers to care throughout the cancer continuum. Provide education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care. Serve as a subject matter expert in navigation, collaborate with program leaders to evaluate community needs, and coordinate high-level care via interdisciplinary rounds and case management. Perform evidence-based practice using the nursing process, including assessment, diagnosis, planning, implementation, and evaluation. Mentor other nurses and staff, participate in shared governance, and maintain competency through mandatory education and data collection. Comply with all Wellstar Health System policies, safety goals, and regulatory standards while functioning in a fast-paced environment.
Required Qualifications
- Licensed registered nurse
- Disease-specific clinical knowledge
- Bachelors Nursing or Accredited Program Nursing or Masters Nursing
- ACLS - Advanced Cardiac Life Support or ARC-ALS
- Amer Red Cross Advanced Life Support or ACLS-I - ACLS - Instructor or ACLS-P - ACLS-Provisional (180 Days)
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- Nationally recognized certification in disease specific area for nurses
- Minimum 5 years nursing practice
- Minimum 2 years nursing with disease specific experience
- Ability to function in a fast paced environment
- Can respond to emergencies in using a decisive, composed and respectful manner
- Possesses excellent time management skills
- Possesses strong critical thinking skills
- Outpatient care or care coordination experience
