ENTRY LEVEL Mental Health Care Coordinator

Job Summary
Entry Level Mental Health Care Coordinator supports outpatient medication management alongside an APRN and Psychiatrist; performs assessments (including substance use, living situation, support systems, mental status, history, strengths/needs, medical status), monitors stabilization through regular contact, coordinates services and referrals, maintains documentation and treatment plans, advocates for individuals and ensures rights are upheld, assesses risk and trauma indicators, and provides family education to promote independence and quality of life. The role emphasizes documentation through Eleos Health, collaboration with the care team, and ensuring compliant, accurate records while supporting care planning and continuity across desktop or mobile access.
Required Qualifications
- Bachelor's degree in Social Work or a related Human Services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of course work includes the study of human behavior and/or development) required.
- Experience working in human services or mental health related field preferred.
- Must demonstrate proficiency in MHRC EHR System within three months of employment.
- Proficiency in Microsoft Office and email are required.
- Must be able to communicate and write English clearly, legibly and correctly at the college graduate level in order to make entries in the records of persons served.
