Job Summary
Entry Level Case Manager working with individuals experiencing homelessness to assess needs, craft individualized case plans, provide referrals to housing and supportive services, support job readiness, assist with housing applications, and maintain HMIS records. The role emphasizes trauma-informed, client-centered care and collaboration with housing providers, with duties spanning assessment, planning, follow-up, documentation, and advocacy. Requires an Associate’s degree and a State of Arizona Fingerprint Clearance Card, valid AZ driver’s license, CPR/First Aid, Naloxone training, and crisis de-escalation training; bilingual English/Spanish preferred.
Required Qualifications
- Associate’s degree in social or behavioral science or related field and minimum of one-year of social services experience, or equivalent relevant work experience.
- Working knowledge of community resources.
- Arizona Level One Fingerprint Clearance Card.
- Intermediate to advanced proficiency in Word, Excel and Outlook; highly accurate data entry skills.
- Arizona Driver’s License and a clean driving record for the last 3 years free of traffic/moving violations, suspensions and/or revocations.
- Must pass Behind-the-wheel certification within 90 days of employment.
- CPR-First Aid certification within 90 days of employment.
- Naloxone (Narcan) training within 90 days of employment.
- Non-violent crisis de-escalation certification within 90 days of employment.
