Job Summary
Enrollment Intake RN for PACE enrollment team; conducts home visits to complete and submit MDS_HC, performs comprehensive intake assessments, discusses needs with the PACE IDT, coordinates additional assessments, and collaborates with IDT to help participants remain safely in the community. Requires MA RN license, 1 year geriatric/mental health experience, strong computer skills, and ability to engage with diverse populations. Full-time, Mon-Fri 8-4, not remote; location Lowell, MA. Flu shot required. Benefits include health/dental/vision insurance, PTO, retirement, and supplemental benefits.
Required Qualifications
- Associate’s or Bachelor’s degree in Nursing
- Licensed as a Registered Nurse in Massachusetts
- Minimum one year experience in geriatric healthcare and/or adult mental health setting
- Advanced computer skills
- Effective communication with medical personnel
- Ability to respond to complex clinical situations
- Strong knowledge of medical/psychiatric diagnosis and treatments
- Experience with diverse populations
- Flu shot required
Desired Qualifications
- Bachelor's degree in Nursing
- Licensed Registered Nurse in Massachusetts
