Job Summary
Enrollment/Intake Registered Nurse (RN) needed to provide care to patients via individualized care plans in a geriatric/home-health context. Responsibilities include conducting assessments to determine eligibility for the Pace Program, performing home visits to see patients in their homes and centers, with frequent local travel and no on-call requirements. Schedule is Monday–Friday, 8:00am–4:00pm, with no nights or weekends and opportunities for a sign-on bonus. Must maintain an active MA RN license and CPR certification; flu shot required, vaccination for COVID not required. Preference for MDS assessment experience and experience working with geriatric populations; strong communication and relationship-building skills essential.
