Job Summary
Enhanced TASC Care Manager responsibilities include conducting screenings and assessments, coordinating linkage to behavioral health treatment and community resources, developing person-centered care plans, and providing intensive care management services for justice-involved individuals with mental health and co-occurring needs. The role involves collaboration with courts, probation/parole, hospitals, behavioral health providers, and other stakeholders to support continuity of care and treatment engagement.
Required Qualifications
- Education: Four (4) year college/university degree in human services, social work, psychology, counseling, or related field.
- Experience: Minimum of two (2) years of experience working with individuals with mental health disorders, serious mental illness (SMI), co-occurring disorders, and/or justice-involved populations preferred.
