Job Summary
Enhanced TASC Care Manager handles professional care management for justice-involved individuals with primary mental health diagnoses within NC’s Enhanced TASC model. Responsibilities include conducting screenings and assessments, coordinating linkage to behavioral health treatment and community resources, developing person-centered care plans, and providing intensive care management services. The role involves collaboration with courts, probation/parole, hospitals, behavioral health providers, and community agencies to support continuity of care and treatment engagement. Requires collaboration across behavioral health and justice systems, proficiency with electronic health records, and ability to communicate effectively; works 40 hours/week, full-time, in Wilmington, NC.
Required Qualifications
- Education: Four (4) year college/university degree in human services, social work, psychology, counseling, or related field.
- Experience: Minimum of two (2) years of experience working with individuals with mental health disorders, serious mental illness (SMI), co-occurring disorders, and/or justice-involved populations preferred.
- Proficiency in electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.).
- Strong communication, organizational, and time-management skills.
- Ability to work independently and manage a caseload effectively.
- Ability to establish and maintain effective working relationships with diverse populations and multidisciplinary teams.
