Job Summary
Coordinate programs and services to help residents maintain quality of life and age in place by assessing needs, identifying outside resources, and networking with community organizations. Initiate psychosocial assessments, interview families, and provide general case management while maintaining updated resident files and completing Care Plans. Engage residents to identify needs, organize off-site programs, and develop a Resource Directory for service providers. Assist in coordinating home care services, counseling, and preventative health screenings, while submitting Semi-Annual Performance Reports to HUD to maintain funding.
Required Qualifications
- Minimum associate's degree or equivalent in social work, psychology, or gerontology
- Two or more years of experience in social service delivery with the elderly and/or family population
- Demonstrated working knowledge of supportive services and other resources in the area served by the project
- Demonstrated ability to advocate, organize, problem-solve, and provide results for the residents served
- Good communication, comprehension and interpersonal skills
- Working knowledge of Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, RealPage, OneSite, or similar related systems, etc.)
Desired Qualifications
- Education: Minimum associate's degree or equivalent in social work, psychology, or gerontology preferred
- Experience: Two or more years of experience in social service delivery with the elderly and/or family population is a plus
