Job Summary
Provide case management to Medicaid Members seeking assistance, including screening, eligibility assessments, and direct service delivery for health-related social needs. Manage incoming referrals to ensure timely connections and develop social care plans summarizing member needs and referred services. Conduct outreach to Members in alignment with required frequency, modality, and timeframe while documenting progress notes and data entry via the SCN client database. Maintain caseload levels through billable activities and coordinate with community agencies, Corps leadership, and Divisional Headquarters. Complete required trainings and represent the organization at community meetings. Full-time role with up to 25% travel to other Salvation Army locations.
