Job Summary
Implement and support individualized patient care plans while addressing risk factors and obstacles to care, including hospital and ER utilization. Facilitate engagement between the primary care physician and patients to encourage active health behaviors, close care gaps, and reduce unnecessary readmissions. Participate in required care coordination conferences, quality gap closures, and multidisciplinary team meetings to address healthcare utilization trends. Maintain a predetermined patient caseload, exercise sound clinical judgment, and work autonomously in a high-volume environment with minimal supervision. Travel up to 75% to healthcare practices in Downtown Los Angeles.
Required Qualifications
- Current Vocational Nurse License
- Two (2) + years of experience in one or more of the following settings, Ambulatory Care Management
- Must have a valid driver's license and be able to travel
- Must be able to lift and/or move between 10 and 25 pounds
- Close vision and ability to adjust focus
