Engagement Specialist - Davita IKC - Cumberland, MD

Engagement Specialist to be the first point of contact for patients, handling onboarding, scheduling, and ongoing outreach to keep patients connected through high-volume, relationship-focused interactions. Responsibilities include CHE completion in nephrology offices or via telehealth, coordinating with Nurse Practitioners and clinics for in-person appointments, making outbound calls for scheduling and reminders, welcoming new patients and addressing concerns, maintaining accurate documentation, and supporting care coordination tasks to move patients through the continuum of care. Must be highly organized with strong verbal and written communication, capable of working autonomously and as part of a care team, and able to operate in a home-office environment with reliable internet. Experience in healthcare or patient-facing roles and proficiency with Microsoft Word, Excel, PowerPoint, and Outlook are preferred.
