Job Summary
Coordinate immediate care for hospitalized youth enrolled in the OhioRISE program by joining families to identify needs, define cultural factors, and identify strengths for engagement. Support discharge processes, educate partners, and make referrals for follow-up care while completing milestones like crisis safety plans, assessments, and Child and Family team meetings. Build partnerships with community resources and ensure services meet safety and treatment needs. Maintain documentation, participate in program evaluation, and lead cross-systems training. Requires three years of experience in behavioral health or related fields, a valid driver's license, high-speed internet access, and cultural humility.
Required Qualifications
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field
- High school diploma or equivalent
- Associate degree or bachelor's degree
- Master's degree or higher
- Valid Driver License
- Ability to operate in an Internet-based, automated office environment
- Ability to operate, receive and conduct video calls and/or meetings in an automated office environment
- Ability to maintain a minimum of 100mbps for high-speed internet connection
- Excellent organizational skills with the ability to stay focused and prioritize multiple tasks
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Willingness to participate in and lead cross-systems care coordination
- Ability to effectively communicate through verbal/written expression
- Culturally humble or responsive with training and experience to manage complex cases
- Qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and who are engaged with one or more child serving systems
Desired Qualifications
- Knowledge and experience in Hi-Fidelity Wraparound
- Two years of experience in a coordinated supportive services or care coordination role
- Experience working with people with autism spectrum disorders and developmental disabilities
- Experience in family systems
- Experience in community systems and resources
- Experience in case management
- Experience in child and family counseling or therapy
- Experience in child protection
- Experience in child development
