Job Summary
Conduct patient assessments and support physicians in care planning and evaluation for chronic conditions, specifically wounds, ostomies, and continence. Manage digital and manual clinical histories, treatment plans, and follow-up photography archives while monitoring stabilized chronic conditions. Execute daily home care routes, respond to user consultation calls, and generate monthly reports for patient audits. Supervise hygiene and infection control protocols during patient care and coordinate in-scope supply delivery.
Required Qualifications
- Professional in Nursing
- 1 year of experience in wound clinic and ostomy
- 1 year of experience in the care of patients diagnosed with neurogenic bladder or intermittent catheterization management
- Ample knowledge of Convatec products and services
- Excellent verbal and written communication skills
- Exceptional customer service skills and ability to multitask under pressure
- Attention to detail and time management
- Ample knowledge of patient care protocols for wounds, ostomies, and continence
- Technical skills and treatment tracking for patients and complication identification
- Demonstrated ability to work in a team
- Digital and/or manual medical history charting for all patients treated at Convatec Medical Care
- Adherence to established norms for patient care
- Daily patient care route established by the home care program coordination
- Treatment plan establishment according to clinical condition following Convatec guidelines and protocols for high, medium, and low complexity patients
- Archiving of patient follow-up photographs in digital folders by insurer, patient, and date
- Response to user consultation request calls
- Projection and reporting of treatment plans for new patients in the outpatient wound or home care clinic program
- Elaboration of patient reports and spreadsheets when required
- Monthly reporting of patients evaluated at home and participation in audits
- Evaluation and supervision of compliance with hygiene standards during cleaning and disinfection tasks
- Evaluation and supervision of compliance with infection control measures during patient care
- Coordination of supply request and delivery to PAD nurses and management of complete records of these actions
Desired Qualifications
- Desirable specialization in auditing with providers or insurers or education
