Conduct territorialization and mapping of the service area to identify individuals at risk, while participating in team meetings and planning local actions. Provide comprehensive health care to the assigned population within the unit, home visits, and community spaces, ensuring integral attention through preventive, curative, and health surveillance activities. Perform active search and mandatory notification of diseases, facilitate user needs through qualified listening, and plan, supervise, and evaluate the work of nursing technicians and community health agents. Maintain accurate health information registration and adhere to institutional norms and ethical standards.
