Implement evidence-based protocols and strengthen safety barriers to mitigate risks and ensure patient integrity. Identify, map, and classify risks in care processes while developing and executing mitigation plans. Continuously monitor critical risks across all units and update the Patient Safety Plan in compliance with RDC 36/2013. Evaluate adherence to international safety goals, including correct patient identification, effective communication, medication safety, and fall prevention. Actively participate in investigations of barrier breaches and serve on medical record and death review commissions. Disseminate a culture of safety and engage staff in safe care practices.
