Job Summary
Assess and review patient and family care needs, manage symptoms, and lead future care planning for life-limiting illnesses. Liaise with community palliative care services, coordinate discharge planning, and educate the clinical team on best practices. Demonstrate advanced skills in health assessment, pharmacological intervention, and interpreting radiology and pathology results. Contribute to service development, quality improvement activities, and data entry for benchmarking. Engage with local stakeholders to inform interventions and lead departmental meetings.
Required Qualifications
- Endorsed Nurse Practitioner (or candidate) with the Nursing and Midwifery Board of Australia and AHPRA registered
- Must have experience practicing at an advanced clinical practice level in the area of palliative care
- Post graduate qualification in Palliative Care
- Minimum of 2 years experience/employment post Nurse Practitioner Endorsement
- or, a minimum of 5 years experience in Senior Palliative Care Nursing roles for Candidates
- Well developed critical thinking skills and experience to determine a safe plan of care
- National Criminal History Check or willing to obtain
- You may therefore be required to undergo mandatory immunisations/vaccinations (including flu vaccinations)
