Job Summary
Develop and monitor service plans, assess individual needs to formulate goals, and provide casework and counseling services while maintaining accurate professional records and communication with families. Guide direct service staff through training and coaching, and act as a medication manager as needed. Participate in the department's on-call rotation to serve as a key resource for individuals, staff, families, and community members. Requires a bachelor's degree in human services or social work with one year of experience, a valid driver's license, and good driving record. Monday-Friday 9am-5:30pm schedule with on-call duties.
Required Qualifications
- Possess current and valid driver's license and auto insurance, and maintain a good driving record.
- Bachelor's degree in human services, social work, education or a related field
- Minimum of one year experience in social services
- Ability to meet employment requirements (i.e. health screening, Driving Record, TB test)
Desired Qualifications
- Experience in a setting with people with intellectual disabilities
