Job Summary
Complete initial and ongoing assessments to identify the physical, psycho-social, and environmental needs of residents. Assess conditions, chart observations, perform evaluations including vital signs and medication reviews, and administer medications as prescribed by the physician. Provide effective communication to residents, family members, team members, and health care professionals. Work the night shift from 5:45 p.m. to 6 p.m. in the new End of Life/Hospice Suite area. Requires current state CNA license, CPR certification, and compassionate demeanor.
Required Qualifications
- Current state license as a CNA
- Proof of current CPR certification
- Ability to pass a background check and fingerprint process
