EMT (Fly Car In Home Assessment)

Conduct in-home assessments for recently discharged patients or those requiring follow-up care without ambulance transport. Obtain vitals, evaluate general condition, and confirm medication access while observing the home environment for safety barriers. Connect patients to the Nurse Practitioner via telehealth when clinical evaluation is needed and document all findings accurately. Identify non-clinical needs to hand off to the Case Management Worker or Social Worker and communicate urgent issues to clinical leadership. Represent the program professionally in all patient interactions to ensure safety and stability post-discharge.
