Deliver comprehensive primary care services to a panel of veterans, managing new and returning patients with conditions such as hypertension, diabetes, and chronic pain. Order diagnostics, medications, and therapies as medically indicated while completing required VA computer documentation and screenings. Implement the Patient Aligned Care Team (PACT) model to coordinate care with multidisciplinary specialists, including mental health and specialty services. Support the Call Center and conduct face-to-face, telehealth, or video visits, with occasional travel to VISN medical centers or Community-Based Outreach Clinics.
