Job Summary

Coordinate intake for new referrals by reviewing materials, conducting comprehensive interviews, and developing Individual Service or Recovery Plans with interdisciplinary team members. Implement these plans to ensure employment readiness and smooth transitions, while developing relationships with community organizations, employers, and family members to secure necessary resources. Empower individuals through self-advocacy activities and facilitate training programs covering job seeking, retention, and relevant life skills like travel training. Review case files to understand individual aptitudes and special needs, then assist in creating professional resumes and arranging interviews with prospective employers. Ensure safe work environments by acting as a resource for employers regarding disability issues, providing crisis intervention, and training individuals on workplace tasks. Complete all required documentation for payroll, progress reports, and service delivery within program data management systems, while managing transportation needs and discharge planning for program transfers.

Required Qualifications

  • Bachelor's degree from four-year college or university in a related field
  • at least one year related experience and/or training
  • equivalent combination of education and experience
  • Valid Connecticut driver's license in good standing
  • Current registration and automobile insurance
  • Ability to obtain approved certification in physical/psychological management training (i.e. PMT)
  • CPR and First Aid certification
