Job Summary
Partner with adults, students, and individuals receiving post-secondary employment services to discover meaningful employment using Customized Employment strategies. Build positive relationships to identify career paths matching unique strengths, then assist with the full job search process including resume creation, application completion, interview preparation, and onboarding. Provide job coaching, individualized training, and ongoing support to encourage natural workplace relationships while partnering with family, employers, and support teams. Develop person-centered employment plans, coordinate referrals, and maintain documentation compliant with 245D Home and Community-Based Services, Vocational Rehabilitation Services, and Extended Employment standards.
Required Qualifications
- Bachelor's degree in a related field
- One year of experience providing direct support to people with disabilities
- Associate degree in a related field
- Two years of experience providing direct support to people with disabilities
- Diploma in a related human services field from an accredited post-secondary institution
- Three years of full-time experience providing direct support to people with disabilities
- At least 50 hours of education and training related to human services
- Four years of full-time experience providing direct support services to people with disabilities under the supervision of a qualified staff person
- Successfully pass a background study required by the Minnesota Department of Human Services and MSS
- Valid driver's license
- Acceptable driving record
- Insurable under the agency's insurance policy
- Reliable transportation to travel throughout the service area
- Comfortable using technology to document services, communicate with team members, and complete required documentation
