Job Summary
Patient Navigator within the Emerging Risk Program provides non-clinical care coordination to patients identified as rising risk, coordinating resources to address SDOH needs (e.g., food, housing, transportation) and supporting goal achievement through coaching and referrals to Clinical staff as needed; works with an interdisciplinary team to ensure compliant documentation, program evaluations, and timely communication while primarily working remote with occasional on-site collaboration in Somerville, MA.
Required Qualifications
- Bachelor's Degree in Healthcare Management or related field required
- 1+ year of social work, case management or related experience preferred, in a clinical setting
- Strong knowledge of healthcare resources, community services, and patient advocacy required
- Strong organizational and time management skills required
- Knowledge of Social Determinants of Health (SDOH) strongly preferred
